Post-acute care

Post-acute care is available for individuals who no longer need hospital care, but who must complete their recovery before they can resume life in the community following injury, illness or surgery. When a person receives post-acute care, he or she enters a highly specialized environment that is responsive and committed to individual healthcare needs.
