If your loved one is coming from the hospital, the hospital will complete a care assessment.

Pre-admission visits from staff
The director of nursing, social worker or a designee will schedule a time to do an assessment to determine if we can meet your loved one's needs. At this time, we will be able to verify all services that should be available upon admission.

Copy of power of attorney – healthcare
Copy of power of attorney – financial
Long-term care insurance policy
Emergency contact information
Advance directives (living will)
Doctor’s orders and medications
We must have signed doctor’s orders prior to admission. If your loved one is transferring doctors, please make sure that all paperwork has been signed and that a history and physical form has been completed by the new physician. We will order from your pharmacy of choice as long as the pharmacy is able to provide bubble packaging for the medications. Approval to use bulk medications is granted on an individual basis.
Personalizing your room
Home is a place to make your own. We encourage you to bring any personal items that will make your room more like your home. Feel free to discuss your wishes with our social worker.
