    Post-acute rehabilitation services can be a critical step to help you transition from the hospital to home as safely as possible.

    Our post-acute therapists will help you recover after a hospitalization

    Post-acute rehabilitation services are for people of all ages who no longer need hospital care, but would benefit from additional therapy and care before returning home following injury, illness or surgery. Our Medicare-certified post-acute rehab programs feature therapy gyms and include physical, occupational and speech therapy.

    When you receive post-acute care here, you'll find 24-hour skilled nursing care, and you'll recover in a comfortable and supportive atmosphere as you work toward your goals.

    And best of all, your rehab therapy team will tailor our services to your individual needs, helping you recover as quickly and safely as possible.

    About our post-acute rehabilitation services

    Our post-acute rehabilitation services offer short-term care following a hospitalization or serious illness. Each post-acute guest is cared for by registered nurses or licensed practical nurses (primary care nurses). After-care may be provided by Good Samaritan Society – Home Care.

    Individuals requiring special therapies, equipment or care to strengthen and adapt to the home environment will find a cost-effective alternative to hospitalization in this program.

    An intensive and comprehensive treatment approach is championed to make sure optimal recovery in a short-term stay.

    Specialty Care Community provides post-acute rehabilitation services in both our young adult and geriatric communities. Our experience in transitional care has indicated that even in the short-term, residents are more comfortable in settings with age-matched peers.

    However, we also recognize that young and old are self-defined, and we strive to accommodate residents’ wishes with regard to appropriateness of placement on an individual basis.
